Our basic premise of care begins with the fact that most women
and many men in America have hormonal deficiencies.
 

 
We test for these hormone levels and other needed values through a
combination of saliva and blood testing. In women, the most prominent deficiency is progesterone and in men, testosterone. Until these deficiencies are restored, patients remain in a state of imbalance. This leads to malnutrition, obesity, fatigue, loss of muscle mass, foggy thinking, to name a few – all leading to accelerated aging.

Once hormone restoration is achieved, our patients/clients are able to follow a healthy diet based on the Mediterranean diet model, reduce stress to the mind and body through yoga and hand work, promote their own emotional and spiritual healing through cinema therapy, and bibliotherapy.

The institute will provide state of the art information and consultation in a model of care to men and women, integrating the following;

1. Hormone evaluation and restoration for perimenopause, menopause,     andropause (male menopause) and adrenal fatigue – through topical,
    plant derived, natural hormones compounded individually according
    to patient defiencies.
2. Nutritional assessment and education in the Mediterranean diet and     specific dietary supplements (such as Vitamin D, Omega fatty acids,
    and minerals)
3. Physical restoration through yoga, message therapy, and handwork     
    (knitting, etc.)
4. Psychosocial restoration through book and cinema therapy
5. Family restoration through meals at home, parent involvement at school     and parent/child physical activities (martial arts, yoga and outdoor     playing)
6. Educational activities on health topics for patients, families, the
    workplace and the community
7. Academic activities including teaching, instruction, precepting, and     mentoring of physicians, pharmacists, medical students, graduate and     undergraduate students pursuing health careers
8. Research which incorporates this model of care for the purpose of far     reaching implementation

The institute strongly recommends patients continue a relationship with their primary care provider, as we do not provide routine primary care. We will, however, plan for a long-standing relationship with patients as we advise and educate throughout their life.

The institute will have a direct “first party” relationship with the patient/client, meaning we do not submit charges to insurance companies. Patients will pay at the time of service with cash, check or credit/debit card. A charge sheet or “super bill” receipt will be generated for patients with private insurance to submit to their insurance carrier for possible reimbursement. The institute will opt-out of Medicare, meaning Medicare beneficiaries may be seen and pay directly, but they may not submit the charge sheet to Medicare for reimbursement. The institute will not be a Medicaid or Workers Compensation provider.

Dr. Stephenson offers a 30 minute visit to review lab work and prescriptions from other physicians/providers, which is separate from our full initial evaluation of $150.

The full evaluation is divided among three components as follows:

1: Initial evaluation - including history, physical exam, waist circumference, hormonal symptom evaluation, depression/anxiety evaluation, nutrition evaluation and life stress evaluation, $300

2: Saliva and Blood testing

- Saliva portion – progesterone, estrogen, testosterone, DHEA, and cortisol x 4 (morning, noon, afternoon and evening), $240

- Blood portion – complete blood count, complete metabolic profile, magnesium, free thyroxin, thyroid stimulating hormone, insulin, IGF-1 Indirect human growth factor measure, cardiac C-reactive protein, lipid profile and PSA in men. -$210.00

-Immune/inflammatory factors; TNF alpha, Interleulin-6& Fibrinogen.
- $250.00

Blood portion, payment option A – Professional component of $60 to Health, Heart and Mind Institute and the remainder billed directly to your insurance provider from Quest Diagnostics, with appropriate co-pays.

Blood portion, payment option B – pay Stephenson Health & Wellness Center directly for all blood work.

3: Follow-up visit - "face-to-face" discussion of lab results, questionnaire results and for devising a long-term plan for healthy aging, $150.

This total amount (initial visit, saliva/blood work, follow-up visit) will be divided into two payments. The total will vary based on lab payment options, and if you have already had recent lab work done.

We will continue maintenance visits (up to $150.00) on a quarterly schedule thereafter. lncluded in this fee are repeat questionnaires as appropriate to follow and track patient/client progress. We will repeat saliva and blood testing as appropriate to follow progress as well (seperate charges will apply)

Compounded natural hormones and dietary supplements will be a separate cost and are not included in the fees noted above.
